Missing All Teeth

This case shows how I replaced patient's missing upper teeth using implants and removable
complete implant bar overdenture.

This patient came to me after the implants were already placed by other dentist. I offered her two treatment options. One was to place more implants in the back and restore them with fixed implant crowns. Second option was to fabricate a removable palateless complete denture using these implants after removing the remaining two premolars. Patient chose second option.

Patient's lower anterior 4 teeth had advanced periodontitis with mobility. It was decided to remove lower front 4 teeth and restore lower arch with a removable partial denture. .

n addition to having periodontal problem, these lower anterior teeth are flared out and prevented me from
setting upper anterior teeth in an ideal location. Another reason why they needed to be extracted.

After the trial denture was verified in mouth, the teeth were indexed in preparation for fabrication of an implant bar.

This implant overdenture bar has three areas where the
denture will clip onto.

Because the maxillary denture will be made with palate open, a metal frame was fabricated to strengthen the denture and prevent fracture during function.

Here is the denture ready to be processed in acrylic.

Two yellow clips are Hader bar clips and the two grey round
things are ERA clips.
Here is the denture ready to be delivered. You can see the
metal housing for Hader clips near the front teeth.

This is the implant overdenture bar screwed onto the implants. Parallel round bar is the Hader bar, and two round rings are ERA attachments.

Occulasl view of the maxillary overdenture. This denture's
palate is open. This allows patient to taste food better, and
the denture is more comfortable to wear.

Mandibular removable partial denture (RPD) in place. A
removable partial denture is an excellent treatment choice for select patients. Partial dentures, when done right, can
deliver comfort, esthetics and function at a fraction of the cost of doing implant crowns.

wanted white teeth. She plans to have her other natural
teeth whitened.
